Plymouth Community Assist

Plymouth Community Assist is a new service in partnership between Plymouth City Council, the Devon Clinical Commissioning Group, and Improving Lives Plymouth.

The aim is to support people who are waiting for occupational therapy and social care assessments by providing a new 'front door' to adult social care in Plymouth.

By helping people to wait well and supporting people to live independent, healthy lives for as long as possible, we can reduce the immediate demand on the health and social care systems.

  • Utility and food provision
  • Sensory needs (hearing and sight)
  • Caring for others
  • Learning disability information
  • Long term health conditions
  • Signposting for free legal advice

Please note that the service is being developed and is not currently open to the public.
