If you are a manager completing this form for a new employee, please enter their details and submit the form on their behalf.

Please note that this form should only be used by people who do not currently have access to Plymouth City Council Adult Social Care systems. This includes Eclipse and the Dashboards. It is possible to request access to archive systems that include CareFirst and the S-Drive but you must have a very good reason and rationale.
