Health visitors are qualified nurses or midwives with additional training in public health. They work in local communities, mainly with children aged up to five and their families, to ensure every child gets the best possible start in life.
In our teams we have a mix of staff who deliver a programme of care to the local community. The teams are made up of Health Visitors, Community Public Health Nurses, and Family Health Workers.
We work with families and predominantly children under the age of 5, supporting in the delivery of the Healthy Child Programme. Our role within the community is to identify health and developmental needs of children and support families at key points in children's early years.
We work in partnership with many other agencies and workers in the community; our aim being to support parents/carers of children to meet their potential. Across the city are child health clinics where families with children can easily drop in and access the Health Visiting service.
Our model of working allows us to improve the care we deliver and how we communicate with key partners, families, carers, local community, GP's, children's centres, other health professionals. We can develop services in response to local need, therefore tackling inequalities at a community level. We work in partnership with communities to identify areas of change and help to build the community capacity to make the changes happen.
