With over 20 years of combined experience within NHS hospitals, private sector treatment centres, nursing homes and in a clients home Southwest Live-In Care is a company that is built on solid principles and understanding of your care needs.
Physiotherapy
Our physiotherapy service is only available in your own home. Southwest Live-In Care specialises in assisting patients back to their full functional capability following orthopaedic surgery, returning home following a hospital admission and balance or falling concerns.
Reablement
Reablement is a short and intensive service delivered in your home, which is suitable for people with disabilities and those who are frail or recovering from an illness or injury.
The purpose of reablement is to help people who have experienced deterioration in their health and/or have increased support needs to relearn the skills required to keep them safe and independent at home.
People using reablement experience greater improvements in physical functioning and improved quality of life compared with using standard home care.
Hospital discharge
Southwest Live-In care can assist with your hospital discharge to ensure that a client is safe within their home environment whilst they are recovering from an illness or undergoing rehabilitation following surgery. Southwest Live-In Care can also arrange for equipment to be installed or delivered to a clients home.
