Supported Living is the term given to encompass a range of care services designed to help disabled or vulnerable people retain their independence in their local community.

It includes people with, for example, a learning or physical disability, acquired brain injury, sensory loss, mental health need, Autism or other eligible health conditions and complex needs.

This service includes all support and care services for people living in Plymouth (Plymouth ASC), and for individuals who are registered with Plymouth GP (CCG).

Supported Living  is for people who have been assessed as requiring and being eligible for a commissioned care and support package in their own home from a Supported Living Framework Supplier who is regulated by CQC (if the care provided is regulated activity). 

This service is predominantly for adults over the age of 18 living within the city of Plymouth (for Adult Social Care funded packages) or has a Plymouth registered GP (for CCG funded packages). 

In addition, there may be some 16+year olds supported by this service who are in transition from children's to adult social care and health services and require support to develop their ability to live independent lives. 

The Service may also be accessed by people with a personal health budget or social care Direct Payment, according to the agreed rates for this service.
