If not then we will involve the families where possible and the multi-disciplinary team to ensure we maintain good working relationships to ensure we meet their care needs. We will support the individuals to have access to ongoing health care and support.
We encourage all of our service users to access and participate in community activities and enabling them to maintain and build good relations within the community.
